Vendor Set Up Paperwork

If you have not already done so you are now required to complete, sign and submit completed vendor set up documents. Refer to documents and directions below. Once you have submitted the following, your company can be issued with a WSI vendor number and can start to receive purchase orders.

Business Contract

You need to complete, initial all pages, sign and return completed contract. Refer to below for which contract you need to submit:

If you are selling off-line products us you need to submit the Master Purchase Agreement (M.P.A).
If you are manufacturing to WSI designs you need to submit the Foreign Manufacturer's Agreement (F.M.A)
If you are selling offline and WSI-designed products you need to submit the Foreign Vendor Agreement (F.V.A)

Product Liability Insurance

You must submit a Product Liability Insurance Certificate issued directly by your insurer in English that meets our minimum cover requirements.

Additional Requirements

Importer Questionnaire - complete this for your factories if 1^st 12 months estimated buy will be less than US$ 100k at cost.
MPA Addendum (Food Vendors only) - complete, sign and submit this as complete document
FDA registration document (Food Vendors only) - submit FDA document showing factory name, address and FDA registration number. Click here for an example of an acceptable FDA document.

Quality Assurance Requirements

Ensure you are fully aware and prepared for your products and protective packaging to pass lab testing. Work with your European QA relevant person if you need guidance. Vendors are requested to pay lab testing as well as supply samples free of charge to the testing lab. In the case that a product fails lab testing and the programme is cancelled as a result, Vendor is responsible for paying lab test costs.

Product testing:

Food items: Not subjected to product testing at Lab
Not food items: Must be subjected to product testing. Merchandising Team will provide the estimated testing costs during the set up process.

Regulatory marking requirements on products by category:

All products must be labeled and manufactured in accordance with global testing protocols and requirements..
